Wellness Program
Description

The Assurance Health & Wellness Center is a program designed to provide adults struggling with serious mental illness the best integrated medical care, and wellness education in the state. Members can receive physician supervised health services, diet and exercise programs, recovery services and peer support, healthy eating and nutrition education, tobacco cessation and stress management, health management and disease education along with health screenings and lab services.

The Assurance Health & Wellness Center is a physician supervised wellness clinic specializing in lifestyle medicine. Through person-centered planning, members will be introduced to health choices including shopping for and cooking healthy foods, importance of an exercise routine, yoga for stress management and the impact of nutrition on chronic diseases. During the nine week program, members will complete a health risk assessment with a primary care physician and develop goals for improved health tailored to the individual health needs of each member to better understand and integrate all components of medical care. An RN Care Coordinator is responsible for coordinating care between the member, physicians and behavioral health providers.

Members attend the program daily for 9 weeks, 6 hours a day. Each day begins with exercise focused on strength, flexibility and balance. Classes include stress management, nutrition, and healthy lifestyle.

Care Coordination-Integration

The Health and Wellness program includes a specific focus on coordinating care with the BHMP and the member’s PCP. Our team is also prepared to participate in all care coordination, meetings, treatment planning, collaboration of care and progress reviews with other providers participating in the member’s care. Health assessments will be shared, as will medication reviews. Progress notes will be included in the primary record. Outcome data will also be shared, such as pre and post testing, blood work, weight and health assessments.

Community Integration, Graduation and Alumni Support

The Health and Wellness program includes field trips such as shopping, museums, parks and the YMCA to help members establish a healthy and independent lifestyle. Members who finish the program receive a certificate of completion and may invite friends and family to celebrate at the graduation ceremony.

Support is available after members graduate. An Alumni support group meets weekly. Drop in refresher classes and alumni specific classes are offered daily.

Orientation

When first admitted to The Assurance Health & Wellness Center, a member will meet with a RN Care Coordinator in order to complete the initial assessments and to help create an individualized and multi-faceted health treatment plan. The Care Coordinator will assist the member in accessing all needed Assurance Health and Wellness services, and will coordinate care with the member’s current primary care provider, and other treatment providers. Members will work with a Navigator, all Certified Recovery Support Specialists, to set health goals based on needs and abilities identified as part of their assessment.

During orientation, each member will meet with a Navigator who is a certified recovery support specialist. The Navigator will work with members to set health goals and ensure wellness plans address both behavioral and physical health needs. Members will meet with a medical assistant who will gather medical status information such as body mass index, blood pressure and other vital signs. Remeasurement at the end of the program will assist members in demonstrating gains achieved from the program. Alumni will talk with members to share information on benefits of the program and offer encouragement and support.

During the first week of their involvement with the Assurance Wellness Clinic, members will be introduced to the overall curriculum, and will have the opportunity to enroll in the aspects of the program that best meet their needs. Members will meet with an on-site physician, an onsite physical trainer and a nutritionist prior to starting any wellness groups.

Primary Care

Primary care will be available on site, and all physician related services will be billed to third party payers, as with any primary care clinic. Wellness Center members are able keep their current primary care assignment or members can opt to be assigned to an Assurance primary care physician and have all primary care services provided at the Wellness Center.
